Parent FAQs

  How will parents be involved with ALERT?
  Parents may receive notifications when their children are due or past-due for shots. They will also be able to obtain complete records of all shots given in Oregon to their minor children. Parents can feel assured that their children will be more likely to receive only the shots that they really need.
  What information are parents able to obtain from ALERT?
  Authorized parents or guardians are able to obtain a copy of their child’s immunization record from the Oregon Immunization Information System. Call 1-800-980-9431 to request an immunization record. Before releasing immunization records directly to a parent or guardian ALERT must receive a signed agreement that the person requesting the immunization record is the legal parent or guardian of the child. The child must be under 18 years old; once a person is 18 they are legally an adult and must make any record requests themselves. ALERT will also forward a copy of the immunization record to the child’s health care provider, school, or child care facility, if requested.
  What age group will be included in ALERT?
  ALERT compiles immunization information on all ages but has the most data on children age birth through 17. The law only allows ALERT to share vaccine information with a person's health care provider, school, daycare, or parent if the person is a minor. Once a person is a legal adult only their health care provider, school or the person themselves may request a copy of their immunization record.
  How will parents be informed that an immunization registry exists?
  ALERT has created a brochure for parents that providers distribute in the clinic setting. ALERT also notifies parents about the registry through a packet of information sent to the parent of each newborn in Oregon. Parents who receive shot reminders will also receive information about ALERT.
  When can a child’s information be removed or restricted from ALERT?
  The law allows authorized parents or guardians to exempt their children from ALERT for health and safety reasons. Contact ALERT directly to find out more about the process to exempt your child, 1-800-980-9431.
